The Resident, a gripping medical drama that aired on Fox starting January 2018, has recently gained attention on Netflix, finding its way into many viewers recommended watchlists. Set within the bustling corridors of Chastain Park Memorial Hospital, the series offers an intriguing behind-the-scenes look at the healthcare system, highlighting the day-to-day lives of doctors, nurses, and administrators.

At its core, the show dives deep into the ethical and personal challenges faced by medical professionals, often portraying dilemmas involving corrupt practices, medical malpractice, and the tension between profit-driven hospital policies and patient care. While fictional, The Resident draws attention to some harsh realities that mirror the real-world challenges of the healthcare industry, one of which is healthcare fraud.

Healthcare fraud is a growing issue that significantly impacts the healthcare sector, causing both financial and non-financial damage. The scale of material losses is staggering, with approximately $455 billion lost annually out of the $7.35 trillion spent globally on healthcare services.

In Indonesia, the situation is equally concerning. According to Alexander Marwata, Deputy Chairman of the Corruption Eradication Commission (KPK), healthcare fraud accounts for about 10% of public health expenditure, translating to roughly IDR 20 trillion in losses. These figures underscore the immense burden fraud places on already stretched healthcare resources.

The sophistication and complexity of healthcare fraud make it increasingly difficult to detect and prevent. Fraudulent activities range from overbilling and unnecessary treatments to more elaborate schemes involving falsified claims and kickback arrangements. Globally, the healthcare fraud analytics market is projected to expand exponentially, with forecasts estimating its value will reach $9.66 billion by 2028

 

Connecting Fiction to Reality

While dramatized for entertainment, the series vividly portrays how doctors and hospital management manipulate patient’s cases, ranging from billing fraud to unnecessary medical procedures.

These scenarios, though fictionalized, reflect common forms of healthcare fraud seen in real life, including:

  • Billing Schemes: Overcharging patients or insurers by claiming for services never provided or upcoding procedures to higher-paying ones.
  • Unnecessary Treatments: Performing excessive or irrelevant medical procedures to inflate costs.
  • Kickback Arrangements: Accepting incentives for patient referrals or prescribing specific drugs and treatments.
  • Credentials Fraud: Employing unlicensed professionals or falsifying qualifications.

 

A real-world fraud case

Just recently, The Malaysian Social Security Organisation (SOCSO), also known as Perkeso, uncovered a significant healthcare fraud scheme. Intelligence gathered by Perkeso’s Anti-Fraud, Ethics, and Integrity department led the Malaysian Anti-Corruption Commission (MACC) to expose a sophisticated operation run by a cartel.

The Jemaah Doktor (JD), or medical board, was arrested on suspicion of colluding with the cartel. For years, this group allegedly exploited false disability claims to siphon millions of ringgits—equivalent to hundreds of thousands of US dollars—from the organization.

Central to the fraud was the practice of upcoding, where doctors submitted false claims with fabricated diagnoses, falsely indicating that claimants had sustained fractures in their hands or legs. To make these claims appear credible, the doctors issued medical certificates granting the claimants 90 days of medical leave.

Moreover, a panel of doctors was complicit in verifying these fraudulent claims. However, upon further investigation, including visits to the hospitals and examinations of actual X-rays, it was revealed that no fractures existed at all.

In this scheme, the claimants received 50% of the money, while the remaining half was distributed among the doctors and agents involved. Perhaps the most distressing aspect of this case is the involvement of medical professionals. Their profession is fundamentally reliant on public trust, and when that trust is broken, the consequences can be far-reaching

 

Combating healthcare fraud

Combating healthcare fraud requires a multi-faceted approach involving prevention, detection, and enforcement. Here are some key strategies to effectively tackle healthcare fraud:

  1. Strengthening Regulation and Oversight

Government and regulators must enforce strict rules on billing, claims, and medical procedures. Regular audits and clear reporting requirements ensure compliance with ethical and legal standards

  1. Whistleblowing Systems

Enhancing transparency and accountability by holding staff at all levels responsible for their actions. Clear and accessible whistleblowing systems, help detect fraud early.

  1. Advanced Technology and Data Analytics

Leveraging technology can help identify anomalies or unusual patterns in claims and billing. 

  1. Education and Awareness

Educating healthcare providers and staff on the legal, ethical, and financial impacts of fraud is key. Regular training raises awareness of common fraud schemes and the importance of ethical practices.

Just as The Resident entertains, it also serves as a reminder of the real-world challenges fraud poses, not only to healthcare systems but in many other forms.

Jakarta, November 28, 2024 – In commemoration of National Fish Day, observed on November 21, 2024, Regal Springs Indonesia (PT Aqua Farm Nusantara) and the U.S. Soybean Export Council (USSEC) hosted a discussion forum titled “Indonesia Tilapia Blue Food.” With the theme “Tilapia: A Source of Protein for Food Security and Global Markets,” the event raised awareness about the importance of fish as a high-quality protein source contributing to both national food security and global markets.

As a producer of premium tilapia committed to sustainability, Regal Springs Indonesia emphasized the importance of collaboration in building public understanding of tilapia’s potential. Tilapia, being rich in protein, was highlighted as a nutritious food choice that supports the development of human resources in Indonesia.

He also noted that tilapia has become one of Indonesia’s top export commodities in the growing fisheries sector. “To meet demand and international market standards, we remain committed to ensuring that our tilapia products are farmed and produced in alignment with good aquaculture practices,” Rudolf explained.

At the forum, Erwin Dwiyana, S.Pi., M.Sc., Director of Marketing at the Directorate General of Product Competitiveness, Ministry of Marine Affairs and Fisheries, emphasized tilapia’s strategic importance.

“Tilapia is the second most farmed fish globally after carp. It is not only rich in omega-3 and other essential nutrients but also makes a significant contribution to meeting community nutritional needs and increasing national revenue,” said Erwin.

Ujang Komarudin, M.Sc., Director of Freshwater Fish at the Directorate General of Aquaculture, highlighted that tilapia is one of Indonesia’s top five export commodities. “This presents a significant opportunity to support national food security while strengthening Indonesia’s position in global markets,” he said.

Prof. Dr. Ir. H. Rokhmin Dahuri, MS, Chairman of the Indonesian Aquaculture Society, shared his insights on tilapia’s role in the global food system. “Indonesia is the world’s second-largest producer of tilapia after China. The tilapia industry has a substantial multiplier effect, from creating jobs to improving community welfare,” he explained.

Bogor, August 12, 2024 – Regal Springs Indonesia (PT Aqua Farm Nusantara) and the Veterinary Research Center – Health Research Organization – National Research and Innovation Agency (PRV BRIN) have signed a research and innovation cooperation agreement aimed at controlling Francisellosis* in tilapia farming. The agreement was signed by Harimurti Nuradji, DVM, Ph.D., Head of the Veterinary Research Center at BRIN, and Sony Sitorus, Director of Regal Springs Indonesia, at the Indraja Building, PRV-BRIN, Bogor.

Director of Regal Springs Indonesia, Sony Sitorus, expressed that Regal Springs Indonesia is greatly honored to establish a strategic partnership with PRV-BRIN. “This cooperation is not only a positive synergy but also a crucial step in realizing Regal Springs Indonesia’s vision to develop technology and innovation that can enhance the quality and sustainability of tilapia farming in Indonesia, as well as the sustainability of the ecosystems where we cultivate tilapia,” said Sony.

This collaboration will focus on the development of molecular detection methods for Francisella noatunensis subsp. orientalis to control Francisellosis in tilapia (Oreochromis niloticus L.) farming, as well as the development of screening methods for Master seed strains as vaccine candidates for Francisella noatunensis subsp. orientalis to manage this disease. In addition, the collaboration will include capacity building and knowledge sharing, as well as the joint utilization of infrastructure and facilities between the two parties.

The Head of the Veterinary Research Center at the National Research and Innovation Agency (BRIN), Harimurti Nuradji, DVM, Ph.D., stated that this collaboration is a preliminary step toward opening up other collaborations.

“In addition, this partnership aims to enhance our capacity at PRV-BRIN, including knowledge, skills, and infrastructure development related to research and innovation in fish health,” said Harimurti.

Furthermore, Harimurti added that this synergy is expected to impact both parties, as well as the broader community, nation, and country, particularly in the control of fish diseases (tilapia).

“In the future, we hope that the results of this collaboration will support the fisheries sector in increasing fish productivity, thereby positively impacting fish production and national foreign exchange,” he expressed.

As additional information, Regal Springs Indonesia is currently implementing various efforts to diagnose and enhance the resilience of tilapia, ranging from clinical symptom observation, sample collection, laboratory testing, to analysis and reporting.

Drh. Juanda, Research & Development at Regal Springs Indonesia, explained that in the process of diagnosis and treatment, all steps taken must always consider the standards set by the Aquaculture Stewardship Council (ASC) and the World Health Organization (WHO).

“We are committed to ensuring that every step we take in tilapia farming adheres to strict international standards to ensure fish health and environmental sustainability,” Juanda added.

This collaboration is expected to contribute significantly to the development of fish health technology and improve the competitiveness of Indonesia’s aquaculture industry in the global market.

 


*Francisellosis is a bacterial disease caused by an infection with the bacterium Francisella noatunensis, which can infect various fish species, including tilapia. This disease often leads to high mortality, especially in young fish or those under stress. The symptoms of francisellosis in Nile tilapia/tilapia vary depending on the severity of the infection and environmental conditions.

Shattering the Silence: Addressing the Culture of Fear Surrounding Whistleblowing

Employee silence often signifies circumstances in which employees withhold critical information that could prevent the organization’s downfall. When something is amiss at work, employees face a tough choice: stay silent or speak up. Sadly, in many cases, they choose not to speak up, fearing that revealing information might be seen negatively or as a threat.

 

The media recently reported that tribunal claims in the USA involving whistleblowing rose by 92% between 2015 and 2023. The number of tribunal cases involving detriment or dismissal following whistleblowing has also increased. This suggests that many whistleblowers still experience retaliation for reporting misconduct.

 

The deaths of two Boeing whistleblowers, currently under investigation, coupled with reports of threats directed at another whistleblower, have heightened apprehension among individuals contemplating the disclosure of misconduct or fraud. Another case is a former Google employee who allegedly faced termination after revealing unethical practices within the tech giant. These cases may only represent the tip of the iceberg, underscoring the gravity of the situation faced by whistleblowers in general.

The Culture of Silence

Many countries have enhanced protections for whistleblowers, with Japan, for instance, recently implementing amendments to its Whistleblower Protection Act (WPA). Despite these governmental efforts to strengthen whistleblower protections, the fear of retaliation may continue to deter employees from coming forward.

This apprehension, which extends to former employees as well, is rooted in a pervasive culture of silence and secrecy in many workplaces, which discourages the reporting of misconduct or illegal activities. The increase in tribunal cases involving detriment or dismissal following whistleblowing can support this notion. The fact that these cases end up in tribunals indicates a company’s failure to establish an effective system for responding to reports due to cultural issues.

Many organizations, whether intentionally or unintentionally, perpetuate a culture of “praise in public, criticize in private,” a mindset that can persist even in the presence of robust whistleblower policy and protection laws. This culture of silence, akin to the fear that speaking out may cause harm, remains prevalent in countries with strong collectivist workplace cultures, such as Indonesia.

In Indonesia, despite the legal framework established by the Law on Protection of Witnesses and Victims since 2006, which provides legal protections for whistleblowers, the underlying culture of silence continues to challenge the effectiveness of these protections.

Breaking the Silence: A Hope for Change

Breaking this culture is such a purge. It may be an uncomfortable and unfamiliar process that takes years and requires a strong commitment, but it will be essential for long-term sustainability.

The change should be initiated from the top management. Leaders must go beyond slogan and actively foster a climate that encourages reporting by enacting policies that protect whistleblowers from retaliation, providing clear channels for reporting violations, and ensuring fair treatment for those who come forward.

Additionally, educating their employees about the importance of whistleblowing and the protection available to them is essential. Government agencies and regulators should continue to enforce laws protecting whistleblowers and hold employers accountable for retaliation against whistleblowers.

This effort should also focus on improving the reporting process and providing support and resources to whistleblowers. While existing laws on protection are significant steps forward for whistleblowers, there is still a long way to go in breaking the culture of silence in the workplace.

Whistleblowers are vital in identifying and preventing harmful practices. It is crucial that they receive protection from retaliation and the support they need to speak out. By working together, we can shatter the silence and create a more transparent and accountable workplace, benefiting everyone involved.

Jakarta, Wednesday 24th July 2024 – The Aceh Singkil District Government, together with Swisscontact, KOLTIVA, and other stakeholders, recently held a Memorandum of Understanding (MoU) signing ceremony about Sustainable Palm Oil Governance 2024 – 202 and launched the Aceh Singkil Multi-Stakeholder Forum (MSF) Dashboard on Sunday, July 7, 2024. Representatives from 12 entities signed the MoU, including the Aceh Singkil District Government, KOLTIVA, Swisscontact, Forest Management Unit VI (KPH VI), Yayasan Inisiatif Dagang Hijau (YIDH), Yayasan Ekosistem Lestari, Forum Konservasi Leuser, Yayasan Hutan Tropis, PT. Sari Dumai Sejati (Apical), PT. Musim Mas, and Aceh Singkils’ Indonesian Palm Oil Association (APKASINDO).

The Leuser Ecosystem and the Rawa Singkil Wildlife Reserve (which includes Aceh Tenggara, Subulussalam, and Aceh Singkil) are vital for environmental protection and represent the last and greatest opportunity to preserve Sumatra’s biodiversity. Despite its protected status, the Leuser National Park has lost one-fifth of its lowland green area due to commercial activities in the past five years. Aceh Singkil District, with the potential 77,512 hectares of palm oil plantations managed by companies and independent smallholders, contributes 31.8% to the district’s Regional Gross Domestic Product (PDRB) in the agriculture, forestry, and fisheries subsector. The district is also home to the Rawa Singkil Wildlife Reserve, which provides a natural habitat for the Sumatran tiger, Sumatran elephant, and Sumatran orangutan.

This multi-stakeholder initiative aims to address sustainable plantation issues and enhance productivity and sustainable palm oil production volume by 30% through intensification over the same period. Recognizing the importance of balancing sustainable economic growth, environmental protection, and social welfare, all parties are committed to ensuring a sustainable future for Aceh Singkil. This initiative highlights the need for a Sustainable Palm Oil Action Plan to position Aceh Singkil’s palm oil as a globally recognized commodity, promoting a greener and more sustainable future for the region.

The MoU provides a framework for optimizing regional growth in Aceh Singkil through accelerated sustainable development among four key pillars: environmental, economic, social, and sustainable governance. The environmental pillar focuses on protecting forest ecosystems and enhancing biodiversity. The economic pillar is dedicated to sustainable land productivity and improving producers’ livelihoods. The social pillar emphasizes inclusive multi-stakeholder engagement to address social inequalities and conflicts. Lastly, the sustainable governance pillar prioritizes structural support and administration for sustainable oil palm action plans.

At the MoU Signing Ceremony event, Drs. Azmi M.A.P., Acting Regent of Aceh Singkil, remarked, “The signing of the Memorandum of Understanding and the launch of the Multi-Stakeholder Forum Dashboard represents the tangible commitment of the Aceh Singkil District Government to establish this area as a sustainable jurisdiction with optimal oil palm productivity and maximum protection of forest ecosystems, particularly the Singkil Swamp Wildlife Sanctuary.”

Swisscontact, as one of the forum members, is committed to creating sustainable production landscapes where economic development coexists with environmental protection and collaborates with KOLTIVA to develop the dashboard.

The Multi-Stakeholder Forum (MSF) Dashboard is an essential tool for monitoring and reporting on the activities of regional stakeholders, facilitating the effective implementation of landscape vision indicators. This platform tracks the progress of MSF initiatives, showcasing the collective actions and efforts of members based on established achievement indicators.

Developed collaboratively by MSF Aceh Singkil members, this dashboard is powered by KOLTIVA, a sustainable farming and supply chain traceability startup in Indonesia. The Badan Perencanaan Pembangunan Daerah (Bappeda) of the Aceh Singkil District Government will oversee the dashboard’s operation through the MSF secretariat. Key features include Robust Reporting Management, where members can monitor key performance indicators (KPIs) across economic, environmental, and social pillars, all presented in an easy-to-read graphical format. It also enables members to submit and report on their activities outputs and their outcomes. The dashboard can also be linked and embedded to external institutional websites, enhancing transparency, and embedded into other platforms for broader accessibility. Additionally, KoltiTrace MIS offers various support functions, including event management for organizing activities and tracking attendance, email blasts for efficient mass communication, and a shared library for documents and videos related to activities.
